1. Online Registration form via Procare
2. Hard copy of the registration form. This form is for OCFS compliance and will always remain on the premises.
3. Medical Form completed by your child’s physician.
4. 1-month deposit, which is non-refundable. If you provide us 2-months notice prior to leaving. In this case, the deposit will be applied as the last month’s payment. The deposit is calculated as four weeks of the rate for the hours you need.
Financial Info:
1. Price varies from $12.50 to $16 per hour2. All payments are nonrefundable
3. All vacations and sick days are paid in full
4. We bill monthly. Our billing cycle is 4-5 weeks
5. In order to change hours, please provide us one-month notice
6. We offer a 10% off siblings discount
7. We charge $10 per 15 minutes for late pickups
8. Late payments of tuition are charged a $40 fee
9. Bounced check fee $30
10. Payments are due on the 10 th of each month
11. We accept payment through Procare, Zelle, Quick Pay, cash or check
In order to get on our waiting list we require a one-time fee of $25 (which can be submitted through Quickpay/Zelle using our email- zusinfamilydaycare@gmail.com). Please note that it does not guarantee a spot, but you will have priority. If a spot opens up, the $25 will go towards your deposit.
